In preparation, please take note that you will need to have the following information to complete your online pre-admission:
Information relating to your admission:
- Date of admission
- Name of admitting doctor
- Facility name
- Short description of the procedure/reason for admission
Personal information of the patient:
- Personal details
- Home address
- Next of kin details
- Other contact person details
- Patient employment details
- An electronic copy of identity document to be uploaded
Account information relating to the payment of the medical account:
- Personal details of the person responsible for the account or medical aid details
- Personal details, postal address and employment information of the main member
- An electronic copy of the medical aid card
